You need to be as accurate as possible when completing the online form, as otherwise your application may be delayed.
Before you start your online application, you need to prepare your identification: we require identification which shows your current name and date of birth. Acceptable forms of identification are scanned copies of your birth certificate, the photo page of your passport or identity card or your photo driving licence.
If your name has changed since you sat your exams, you must also provide a scanned copy of your marriage certificate, deed poll or Decree Absolute.
All identification must be in English and your scan must be in an image format (jpg, png or gif) – PDFs cannot be uploaded.

The costs for replacement certificates and certifying statements of results are £43 per series (a series is a set of exams taken in the same period, e.g. all your GCSEs taken in one summer or winter sitting). The costs are calculated within the online application.
If you submit an application and we are unable to find any results for you, or you do not require the subject(s) and results we find, we will refund 50% of the fee charged.
If you wish to cancel your application, you have five working days from the date of submission in which to do so. Please email your cancellation request to GQHistoricalRecords@ocr.org.uk. We will refund your search fee and any other additional payments you may have made.
